    Abstract
During the last 25 years, a small number of meaningful DNA-based diagnostic tests have been available to aid in the diagnosis and subsequent treatment of heritable disorders. These tests have targeted a limited number of genes and are often ordered in serial testing strategies in which results from one preliminary test dictate the subsequent test orders. This approach can be both time and resource intensive when a patient requires several genes to be sequenced. Recently, there has been much discussion regarding how 'massively parallel' or 'next-generation' DNA sequencing will impact clinical care. While the technology promises to reduce the cost of sequencing an entire human genome to less than US$1000, one must question the diagnostic utility of complete genome sequencing for routine clinical testing, given the many interpretive challenges posed by this approach. At present, it appears next-generation DNA sequencing may provide the greatest benefit to routine clinical testing by enabling comprehensive multigene panel sequencing. This should provide an advantage over traditional Sanger-based sequencing strategies while limiting the total test output to sets to genes with known diagnostic value. This article will discuss the current and near future state of clinical testing approaches and explore what challenges must be addressed in order to extract diagnostic value from whole-exome sequencing and whole-genome sequencing, using hereditary colon cancer as an example.

The most disturbing aspect of Bob Doles ad campaign is its sub-text. In effect, he is telling men not to worry about the prospect of surgery because now theres a drug that will solve E.D., should that be their fate. He would have better served his fellow men by encouraging them to think twice before allowing any doctor to order a PSA test. "I have that conversation a lot with the older gentlemen, particularly those guys over 75 years old," Mason said. "Those are the guys who really have to weigh whether it's worth it." Numerous alternative therapies are used to improve sexual function. Some include: niacin, zinc, copper, Korean red ginseng root, ginkgo, pine bark, Tribulus terrestris, arginine, Avena sativa, horny goat weed, maca root, muira puama, saw palmetto, and Swedish flower pollen. None of these however have been recognized as effective by the FDA.[30] While zinc deficiency may be a cause of lower testosterone levels in hemodialysis patients, which may benefit from zinc supplementation,[31] such supplements have no effect on the testosterone levels of healthy males who consume a zinc-sufficient diet.[32] Vascular reconstructive surgery Restores natural erections when successful An inflatable penile prosthesis consists of two soft silicone or bioflex (plastic) tubes inserted in the penis, a small reservoir implanted in the abdomen, and a small pump implanted in the scrotum. To produce an erection, a man pumps sterile liquid from the reservoir into the tubes by squeezing the pump in the scrotum. Cardiovascular diseases: The most common cause of cardiovascular diseases in the United States is atherosclerosis, the narrowing and hardening of arteries that reduces blood flow. Atherosclerosis typically affects arteries throughout the body and is aggravated by hypertension, high blood cholesterol levels, cigarette smoking, and diabetes mellitus. When coronary arteries (arteries that supply blood to the heart muscle) are narrowed by atherosclerosis, heart attacks and angina occur. When cerebral arteries (arteries that supply blood to the brain) are narrowed by atherosclerosis, strokes occur. Similarly, when arteries to the penis and the pelvic organs are narrowed by atherosclerosis, insufficient blood is delivered to the penis to achieve an erection. There is a close correlation between the severity of atherosclerosis in the coronary arteries and erectile dysfunction. For example, men with more severe coronary artery atherosclerosis also tend to have more erectile dysfunction than men with mild or no coronary artery atherosclerosis. Some doctors suggest that men with new onset erectile dysfunction should be evaluated for silent coronary artery diseases (advanced coronary artery atherosclerosis that has not yet caused angina or heart attacks). This type of implant has no inflation mechanism but provides adequate rigidity to the penis to allow penetration. Nonpharmacologic options may be offered as second-line treatment in lieu of intraurethral or intracavernosal injection for patients who do not respond to or cannot tolerate oral therapy. Vacuum erection devices increase corporal blood flow, and a constrictor ring is then used to retain this blood within the penis. Satisfaction is variable (27%–74%), and this technique can cause discomfort and bruising of the penis.63 Surgical options exist for patients with erectile dysfunction. Penile arterial bypass surgery is appropriate in only a select group of patients (men under 35 years of age who have no generalized vascular disease and in whom an isolated injury has obstructed blood flow). Penile implants are available for patients who have not responded to more conservative treatment. This procedure is invasive, irreversible and subject to complications such as infection, erosion and mechanical failure. There is, however, a high rate of patient and partner satisfaction.63 Mechanical failure leading to re-operation and removal
